Leonardville Nursing Home is each elder’s home. We are committed to providing individualized care by creating meaningful lasting relationships between staff and residents. Neighborhoods

Leonardville Nursing Home is not a traditional nursing home, but rather three small neighborhoods that foster individualized care for elders in a home-like environment. Each neighborhood has versatile workers consisting of a clinical coordinator, neighborhood coordinator, direct caregivers, life enhancement director and a homemaker

Relationships

We provide consistent assignment of self-led teams which means you or your loved one will have the same caregivers consistently.

Memory Care

We provide a safe and secure environment for you or your loved one while promoting dignity and respect for those with dementia and Alzheimer’s.

Daily Routine

Elders decide how they spend their day.
Team members work together to discover and support what gives each elder meaning and pleasure.
Elders are involved in planning formal activities as well as in determining daily spontaneous activity.
Different activities are conducted simultaneously in each of the neighborhoods, allowing individuals to choose activities they enjoy most.
Larger group activities, involving all neighborhoods, take place in the Community Room.
Individual spiritual and cultural preferences supported.
Bereavement teams offer end of life support to elders and families.
